Jake was born in Liverpool on February 26, 2001. Jake was diagnosed with a rare neuro transmitter disease SSADH at the age of 9 months. Sadly Jakes condition deteriorated meaning he needed 24 hour care. During his life he was looked after by a team of fantastic carers and doctors and nurses from Alder Hey. He passed away peacefully in the arms of his family on 15th April 2013. He is much loved and will always be remembered by all his family and friends. His memory will live on forever.
